Child Support Owed

Under Section 51.70 (a) (8) of Title 22 of the Code of Federal Regulations, a passport, except for direct return to the United States, will not be issued in any case in which the applicant has been certified by the Secretary of Health and Human Services as notified by a state agency to be in arrears of child support in an amount exceeding $2,500.00.  For more information, click here.

Your passport will be revoked
if you fall under this regulation, including when you apply for additional pages, seek a renewal or process a Report of Birth application.

Please direct any questions to the appropriate State child support enforcement agency. You may go to the Department of Health and Human Services - State Child Support Enforcement Web Site for a listing of HHS state and local agencies.

Adding Pages to Your Passport
If your full-validity passport has not yet expired, but has no blank pages left, we can add additional pages for you free of charge.  This service can take a few days to process, so please plan accordingly.

We can only add two sets of pages; after that, you will need to replace the passport even if it has not already expired.

We cannot add additional pages to a limited validity passport. Limited passports have a duration of less than 10 years (for adults) or less than 5 years (for children).  If you run out of blank pages in a limited validity passport, you must apply for a new, full validity passport. Details on how to apply are here.  

Application Form for More Pages
Please complete an application form DS-4085.  Download it here. These forms and others are also available in our offices.  Note that the online version of this form includes instructions on the back side written for use in the United States which do not apply to your application in Nicaragua.  (You will need to use free Acrobat software to view and print this form.  You can download the software.)

This form must be completed and signed by the passport holder him/herself.

Changing Your Name in Your Passport
If you would like to change your name in your passport to reflect a marriage or legal name change, you will need to apply for a new "replacement" passport.

Here's what you'll need:

Application Form
If you apply to have your name changed within one year of your current passport's issuance date (i.e. your current passport is less than one year old), you should submit a "U.S. Passport Re-Application Form" (DS 5504), together with the passport that needs the change, the documentation required (see below) and one 2 inch by 2 inch (5 cm x 5 cm) passport photo.

Note that the online version of the application form includes instructions on the back side written for use in the U.S. which do not apply to your application in Nicaragua.

If you apply to have your name changed one year or later after your current passport's issuance date (i.e your current passport is more than one year old), you will need to submit a passport application (DS-82 or DS-11) and pay the full fee for a new passport: $75 for adults, $85 for children. You will also need to send us the passport that needs the change, the documentation required (see below), and one new 2 inch by 2 inch (5 cm x 5 cm) passport photo.


Documentation
You'll need to show us legal documentation supporting any requested change. For example, your original marriage certificate showing your new spouse's name or an original court order showing a legal name change. We cannot accept copies, and we reserve the right to request additional documentation.


Correcting Printing Errors in Your Passport
If your passport contains incorrect data about you (such as a misspelled name, incorrect date of birth or incorrect gender) because of a printing error made by Passport Services, you should act as quickly as possible to have the error corrected.

If you're applying to correct an error within one year of the passport's issuance date (i.e. the passport is less than one year old) you should submit a "U.S. Passport Re-Application Form" (DS-5504), together with the passport that needs the change and one 2 inch by 2 inch (5 cm x 5 cm) passport photo.

If the passport you wish to correct is MORE than one year old, you will need to submit a passport application (DS-82 or DS-11), together with the passport that needs the change and one (2) 2 inch by 2 inch (5 cm x 5 cm) passport photo.

Note that the online version of the application form includes instructions on the back side written for use in the U.S. which do not apply to your application in Nicaragua.

To correct an error in your passport you will need to provide original (no photo copies will be accepted) documentation showing that the information in the passport is incorrect. This documentation might include your birth certificate, your marriage certificate, appropriate court orders and/or other forms of government-issued identification.

Of course there will be no fee to correct an error if the fault lies with Passport Services.


Replacing a Limited Validity Passport Issued for Emergency Travel

Limited Validity passports that cannot be extended are issued for a variety of reasons, but usually are designed to cover emergency travel. Such passports, valid for a maximum of twelve months, can only be replaced with a full validity passport.

To obtain a new full validity passport you should apply using the "U.S. Passport Re-Application Form"(DS-5504) along with the limited validity passport, the letter which you should have received at the time you obtained the limited validity passport, and one new 2 inch by 2 inch (5 cm x 5 cm) passport photo. Please follow the requirements as explained here.

If you trade in your limited validity emergency passport before it expires, there will be no fee to have it replaced with a full validity passport. If you wait until after the limited validity passport has expired, you must use form DS-82 (or DS-11 as appropriate) and you will be charged the full fee to have it replaced.

We cannot issue a second limited validity passport to replace an earlier one; you are expected to provide the information needed to enable us to issue a full validity passport following the issuance of a limited one.

Because of these potential delays, you should not make any unchangeable travel plans until we are able to hand you your new passport.
